UVP w/ Ring Central - Call History Multiple entries

We use the UVP (App Version 5.0.14.660) with Ring Central SIP, each call will usually show up 4 times in the call history ("recents") -- while the call is inbound and unanswered, it will look like you are getting 4 new calls.  Is there any known way to fix this?

Have you tried to reset the phone and reprovision?
